The effective date is the date that we will cancel your service(s) with us. Our standard policy is to have this date 30 days from the date this form has been signed. However, if you put an “effective date” that is less than 30 days understand that you have chosen to forego the 30 days of service you paid for. We will not reimburse any funds for having us disconnect services paid for early. We will cancel all service(s) on the Effective date regardless if you have paid for the 30 days.

By submitting this form, I authorize Post Alarm Systems to make change(s) to my alarm system monitoring permanent records. I hereby declare that I am the authorized principle of the account for the above requested changes and have provided the account Codeword/password for verification in this submittal.
